Lumpy/Sunken Looking Roof

Recently put an addition on and the roof and slowly gotten a lumpy, sunken look. Has anyone ever seen something similar? what is typically the cause? OSB was used for the decking, nails and clips.

Not sure whether you posted a photo or not but it is not appearing in the message. OSB get wet prior to your underlayment?  Did you use 30# felt or synthetic underlayment?  Did you meet the sheets on a rafter or between a rafter?  Was you underlayment flat when you applied your singles?   What weight shingles did you use?  Did you use 5/8" or 1/2" OSB sheeting or plywood?   Were your rafters set on 16" or 24" OC.  Did you use factory made rafters or did you make them up yourself.  All or a combination can contribute to what you describe.
