Sounds like the Glide House to me Mark.  Goggle that or think there's info on Sunset Magazine's site.

Sunset Magzine did a special back in May on a modular house---had it on display at their headquarters.  They wrote a fairly large article about it and have a brochure that you can download laying out various flooplans.

Cool looking houses.  Boy are they expensive!  And big too!

Check out http://www.shelter-kit.com/unitone.html too. Have a look around - there's more than immediately meets the eye.

Instead of buying kits, you could use John's plans to do the same sort of thing.

Here a couple more. These houses have been designed for the tropical north. The are mostly steel frame and have large openings. They use sliding screens, as the whole house is open most of the time. This is for cross flow ventilation and to catch the cooling breezes.

Cute.

Definitely within the 14 ft wide range for the Shax.

While I was poking around on the net yesterday, I found this, maybe a starting point for a small house plan.
